Hazel Annette Ledbetter Mathis was born September 30, 1934 in Scranton, TX to Clara May and Ralph Ledbetter. She was preceded in death by husband, Leon Mathis; son, Darrell Mathis, her parents; sister, Carolyn Wooten, brother in law, Don Wooten and nephew, Ricky Wooten.

She was the eldest of two daughters. She married the love of her life Elbert Leon Mathis on June 2, 1951.
Leon and Annette had four children. Both Annette and Leon were graduates of Northside High School in Fort Worth. Answering the call of country life they moved to Springtown in 1964. They were very involved with their children’s activities and the community. Annette was a devoted wife and a loving mother and grandmother. She had a large extended family of aunts, uncles, cousins, nieces and nephews whom she loved dearly. For her, family was everything. She will be missed by those who knew and loved her.


She is survived by her daughter, Debra Cozart husband, Larry, daughter, Beverly Leatherwood husband, Maynard, son, Barry Mathis wife, Taressa and daughter in law Sheri Mathis; grandchildren, Brandon and Sarah Cozart, Christi and Bryan Schmidt, Chance and Bre Cozart, Lea Ann and Jeff Clewis, Marty and Paige Leatherwood, Marcus Mathis, Macey and Casey Melton, Myranda Mathis, David and April Young, Derek Young, Mark and Summer Mathis, Bret & Stephanie Mathis, Mallory Myers; great grandchildren, Keegan, Hudson & Rowan Cozart, Grayson and Layne Schmidt, Leon Cozart, Celeste Conlin, Zack and Levia Leatherwood, Grant and Elle Clewis, Zayne and Zaylee White, Jayce, Jett and Callan Melton, Travis, Trevor, Trinity and Violet Young, Taylor Ervin, Brock and Carli Mathis, Brooklyn and Dalton Mathis, Kellen and Baylor Gradisar, Brylie, Jack and Tatum Myers; niece, Connie Hencke husband, Art, nephew, Dusty Wooten wife, Sandy; cousins, Ann Hamilton, Richard and Tanya Snoddy, Diane and Rick Newton, Sandy and Pam Ledbetter, Larry and Peggy Ledbetter, Betty and Doug Sistare, Patricia Tudor, Kenny and Nancy Brummett, Leddie and Monty Martin; many great nieces and nephews. Her loving caregiver for many years, Nyla Denise Bryant and the loving staff at Hillside Assisted Living.
